Welcome to Care Suite Healthcare. These Terms and Conditions ("Terms") govern your use of our services and your stay at our assisted living facility. By using our services, you agree to comply with and be bound by the following terms.
1. Acceptance of Terms
By choosing to receive services from Care Suite Healthcare, you agree to be bound by these Terms. If you do not agree to these Terms, you should not use our services.
2. Services Provided
Care Suite Healthcare provides assisted living services, including but not limited to:
- Personal care assistance
- Medication management
- Housekeeping
- Meals and nutrition services
- Health and wellness programs
Specific services will be outlined in your individual care plan, developed in consultation with you and your designated representatives.
3. Admission and Discharge Policy
Admission to Care Suite Healthcare is subject to assessment and availability of resources. We reserve the right to decline admission based on our evaluation of your needs, safety, or other factors.
Discharge from our facility may occur if:
- You require care beyond the scope of our services.
- There is non-payment of fees.
- You or your family violate the rules and regulations of the facility.
- Your behavior poses a risk to other residents or staff.
4. Resident Responsibilities
Residents and their representatives agree to:
- Provide accurate information regarding health, medical history, and any other information required for care.
- Adhere to the rules, policies, and procedures of Care Suite Healthcare.
- Treat staff and other residents with respect.
- Report any concerns about care or safety to facility management.
5. Health and Safety
We take reasonable steps to ensure the health and safety of residents. However, Care Suite Healthcare is not responsible for:
- Any injuries or health conditions that are not directly caused by the negligence of our staff.
- Deterioration in a resident's condition due to pre-existing health conditions.
- Medical complications arising from a resident's refusal to follow prescribed treatments or care recommendations.
6. Medical Services
We provide assistance with medication management and coordination of medical services. However, Care Suite Healthcare is not a medical facility, and we are not responsible for diagnosing or treating medical conditions. You are responsible for arranging visits with your healthcare providers as needed.
7. Personal Belongings and Valuables
Residents are encouraged to only bring necessary personal items. Care Suite Healthcare is not liable for the loss, theft, or damage of personal belongings, including but not limited to cash, jewelry, electronics, or other valuables.
8. Fees and Payment
All fees are outlined in the service agreement and must be paid according to the agreed-upon schedule. Late payments may result in additional charges or discontinuation of services. Failure to pay fees in full may result in discharge from the facility.
Refund Policy
Refunds will be issued on a case-by-case basis and only for pre-paid services not rendered due to unforeseen discharge or termination. Refunds will not be given for voluntarily unused services.
9. Privacy and Data Protection
Care Suite Healthcare is committed to protecting the privacy and confidentiality of your personal and medical information in accordance with applicable laws. We will not share your information with third parties without your consent, except as required by law.
10. Limitations of Liability
To the fullest extent permitted by law, Care Suite Healthcare, its staff, and affiliates will not be held liable for any indirect, incidental, special, or consequential damages arising from your stay at our facility, including but not limited to:
- Injuries, accidents, or loss of personal property not caused by our direct negligence.
- Changes in your health condition not due to the direct actions of our staff.
Indemnification
You agree to indemnify and hold Care Suite Healthcare harmless from any claims, damages, or expenses, including reasonable attorney fees, arising out of your stay, except where such claims are due to our gross negligence or intentional misconduct.
11. Use of Facility Property
Residents are responsible for maintaining the property provided to them by Care Suite Healthcare, including furniture, equipment, and common areas. Damage to property caused by willful misconduct or negligence will result in charges for repair or replacement.
12. Visitation and Guest Policy
Visitors are allowed during designated hours and must comply with the facility's rules regarding visitation. Care Suite Healthcare reserves the right to restrict or deny visitation in cases where it may compromise the safety, privacy, or well-being of residents.
13. Changes to Terms and Conditions
Care Suite Healthcare reserves the right to modify these Terms at any time. Any changes will be posted on our website and communicated to residents as necessary. Your continued use of our services after such changes constitutes acceptance of the new terms.
14. Governing Law
These Terms are governed by and construed in accordance with the laws of Hennepin County, Minnesota without regard to its conflict of law principles.
15. Dispute Resolution
Any disputes arising out of or in connection with these Terms or your stay at Care Suite Healthcare will be resolved through mediation or arbitration in Hennepin County, unless otherwise required by law.
